I confirm that all of the information provided in this application is true and correct to the best of my knowledge and is being relied upon by Penninghame Foundation in deciding my acceptance onto the process.

I understand and agree that any failure to answer any of the above questions fully and accurately may result in my termination from the Process if discovered, and will relieve Penninghame Process from any responsibility for failing to address the undisclosed condition(s). I understand that participation in the Process is entirely voluntary. I am completely responsible for applying for and participating in the Process. If at any time, I determine that continuation in the program may result in any harm of any kind to me, I will immediately advise a member of staff and, if deemed to be appropriate or necessary, withdraw from the process.

I confirm that I will share any changes to this information which arise between now and the beginning of the Process.

I acknowledge that if any of these changes would have led to us not accepting your application, we shall have the right to decide that you cannot attend the course.
